Conflict-of-Interest Checklist
Mirrors Google Form field 11 (multiple choice: “I confirm” / “I do not confirm”). Required.
The COI definition is in Appendix A of the RFP PDF (page 18+). Until that section is transcribed verbatim into this page in Phase 2, the checklist below is a working version aligned with standard NSF-style COI rules. Re-verify against the PDF before submitting.
Working COI checklist (per reviewer)
For each suggested reviewer, confirm none of the following apply within the COI lookback window stated in PDF Appendix A:
- Same institution / department as PI or any Co-PI
- Co-author on any paper, preprint, or grant in the past N years (N per PDF)
- Current or former PhD advisor / advisee / postdoc supervisor / supervisee
- Current or recent (within window) collaborator on funded research
- Direct family member, spouse, or domestic partner of PI or Co-PI
- Current or recent financial relationship (consulting, equity, paid speaking, etc.)
- Current dispute, formal complaint, or known personal animus with PI or Co-PI
- Holds a position that gives them direct authority over PI’s career (chair, dean, funding officer for the proposal)
If any box is ticked for a candidate, drop them and pick another.
